Before you load up your bike and set off, there’s one important piece of admin you can’t overlook: getting a medical certificate signed by a doctor. Without it, you won’t be able to take part.
This guide explains everything you need to know about the Tuscany Trail medical certificate — from official requirements to the fastest way to get yours signed online by a qualified sports doctor.
The Tuscany Trail is the world’s largest bikepacking event, attracting more than 600 riders each year. It isn’t a race — there are no rankings, no stopwatch, and no support vehicles. Instead, it’s a one-stage, self-supported adventure of around 430–440 km with 5,000–6,000 m of elevation gain, weaving through medieval towns, forests, vineyards, and along the coast.
The event follows a “no race” philosophy. You decide when to stop, where to sleep, and how hard to push. Many riders camp at the official base camps along the route, others book B&Bs or simply ride through the night.
For newcomers, there’s also a shorter 160 km route designed to give first-timers a safe but equally scenic introduction to multi-day bikepacking.

To take part in the Tuscany Trail 2026, every rider must present a valid competitive medical certificate for cycling. This is a mandatory condition under Italian regulations — without it, your entry will not be validated, you won’t receive your race number, and you will not be entitled to a refund of the registration fee.
The Tuscany Trail is no easy spin. Over 440 km (or the shorter 160 km route), you’ll be tackling long gravel climbs, rutted forest tracks, cobbled village streets, and rolling roads. Some days it’s blazing sunshine and dust, other days you’re fighting mud and rain — either way, the effort adds up fast.
This isn’t just about fitness. You’ll be riding self-supported, often on little sleep, carrying your own kit, and pushing your heart, lungs, and legs well beyond a normal weekend ride. For anyone with asthma, blood pressure issues, or an old injury, those stresses can become a real problem once you’re deep into the event.
That’s why the organisers — and Italian law — insist on a sports medical certificate for competitive cycling. It’s not box-ticking. It’s confirmation that you’ve had a health screen and are fit enough to handle the demands of a multi-day adventure like this.
You can upload from 10 December 2025. The deadline is 15 April 2026.

The Tuscany Trail 2026 will take place from 20 – 27 May 2026. Riders will once again set off from Campiglia Marittima (LI), a hilltop town in southern Tuscany that has become the traditional launch point for this legendary bikepacking adventure.
