Yes. Under Italian regulations (and per the official rules of the VDA Trailers / TORX® organisation), all runners in Tor des Géants must submit a valid “sport medical certificate” attesting to their fitness for competitive trail running.

Yes — Tor des Géants provides its own official medical certificate template each year. The form confirms that you have “no contraindications and are suitable for the competitive practice of trail running” and must be completed, signed, and stamped by a licensed doctor.
You can normally download the latest version from the event organiser’s website once released, or request a copy from them directly. Runners living outside Italy usually need to use the specific template for foreign participants, which must be uploaded before the registration deadline.
Tor des Géants is an extremely demanding event — 330km, 24,000+ metres of climbing, variable weather, long nights, and severe fatigue over several days. The mix of altitude, continuous exertion, dehydration, calorie deficit, and sleep deprivation pushes the body well beyond normal limits.
For runners with underlying conditions — such as high blood pressure, asthma, heart rhythm issues, or a history of chest pain or surgery — these stresses can turn a manageable problem into a serious medical event, often far from immediate assistance on remote mountain terrain.
